Yes. We will need a copy of your most recent insurance card each time you come in for service. This ensures timely and accurate billing to your insurance company. Insurance changes are not uncommon, therefore, it is important to have an up-to-date copy of your insurance card so that we have correct claims filing information.

Some patients may not pay anything out of pocket. Others will be responsible for the co-pay or deductible prior to being treated by a provider. The amount owed is often indicated on your insurance card, however you can contact your insurance company for details regarding co-pay or deductible amounts.

Yes, we will submit claims to secondary insurance after your primary insurance has processed your claims.
If you don't have health insurance, we offer affordable self-pay option of $85 per visit for immediate access to care. Certain medical devices, labs, and pharmaceuticals may incur additional charges. Vaccinations, drug screens, and physicals are priced separately. Any additional fees will be communicated upfront for collaborative decision-making between you and your provider.
We recommend scheduling a routine check-up annually for most patients. However, for patients with certain chronic conditions or at higher risk for certain health issues, we may recommend more frequent check-ups.
Please call our office during regular business hours to request a prescription refill. We will need to know the name of the medication and the pharmacy you use.
We offer a range of diagnostic tests including blood tests, urinalysis, EKGs, Ultrasound and more. Our providers will recommend specific tests as needed based on your individual health needs.
These physicals are not typically covered by your insurance. Also keep in mind that they are not a substitute for a comprehensive medical exam by the patient's primary doctor. The fee for sports physicals (high school age & younger) is $35.00 and excludes any lab work. If lab work is necessary, whether in-house or off-site, additional fees may be incurred but you will be informed of any additional fees before the labs are completed. The fee for other physicals varies, please contact us for the most up-to-date pricing.
